Eligibility

In order to become a patient at our clinic, clients must first meet Ryan White eligibility criteria.

At the initial appointment, the following documents are required to qualify for services:

• Proof of HIV status
• Proof of income
  • Must span the last month (4 weeks)
  • Can be in the form of check stubs or award letter
• Proof of address – 1 form
• Picture ID

Patients may continue to receive services at the Specialty Care Clinic if they maintain their eligibility by updating their financial and residential documents every 6 months and attend their regularly scheduled lab and medical appointments.

Fees for all services are charged on a sliding scale depending on client’s income. Clients must present all forms of insurance in order to receive care. Medicare, Medicaid and various private insurances are accepted.

Please be aware that the appointment will be rescheduled if required documentation is not brought in for the initial appointment.
